Pay: $2,560 to $2,720 per week Shift Information: Days - 5 days x 8
hours Contract Duration: 13 Weeks Start Date: ASAP About the
Position Pay Rate: $64.00 - $68.00/Hourly. Summary: We are looking
for a licensed Physical Therapist to join our team of health
professionals in providing excellent health care in our facility.
Typically, you'll be working with post-op patients to help them
with recovery. You will be consulting with them to learn about
their current condition and assess their patient evaluations in
order to develop proper treatment plans. Responsibilities: Evaluate
and treat residents in skilled nursing and post-acute settings.
Develop and implement individualized treatment plans to help
residents achieve their highest functional potential. Collaborate
with an interdisciplinary team to ensure seamless, coordinated
care. Document accurately and timely in accordance with facility
and regulatory standards. Participate in care conferences and
contribute to ongoing quality improvement efforts. Requirements
Requirements: 2 - Year experience Required. Active California
Physical Therapist license (required) BLS Certification. Benefits
